Dear HoG:
I recently received a mailing from you called the “Client Medication Profile.”  Why are you asking me to complete it and mail back to you?  I get my child’s other non-hemophilia medications from my local retail pharmacy.

Dear Concerned Mom:
The pharmacy must keep a current record of drug allergies and a list of all medications prescribed for each client.  To do this we ask every client to complete and return the Client Medication Profile once a year.  It is important to list all current medications, prescriptions and over-the-counter items your child is currently taking as well as to list drug allergies and/or a latex allergy.   Our pharmacist reviews these forms and looks for potential problems.

Remember to always tell each of your doctors about all the medications you take and about any changes in your medications prescribed by other physicians.  Please feel free to make copies of the Client Medication Profile to give to your physicians and other pharmacies.  The Client Medication Profile form is also on the website, under www.hog.org/MedForm.
