- If you have hemophilia, what percentage (%) of factor do you have in your blood?
- Do you have an inhibitor from taking factor concentrate?
- If you use factor concentrate, what brand is it and what is the dosage? How often do you take it?
- What other medication do you use to help control bleeding symptoms (name, dose, and when do you take it?)
- Have you had the shot (vaccine) for Hepatitis A, or are you immune?
- Do you have Hepatitis B, have you had the shot (vaccine), or are you immune?
- Name of shots (vaccines) you have had or will need in the future
- If you have Hepatitis C
- If you have HIV
- Condition of your joints
- What are symptoms you watch for with bleeds?
- Family's bleeding disorder history and who is a carrier
